Already after using 2 g of gel, a decrease in the secretion of adrenocorticotropic hormone (ACTH) by the pituitary gland may occur through inhibition of the pituitary-adrenal system.

The mechanism of the anti-inflammatory action of fluocinolone acetonide has not been fully studied, and it is generally accepted that this medicinal product reduces inflammatory processes by inhibiting the production of prostaglandins and leukotrienes as a result of inhibiting the activity of phospholipase A-2 and reducing the release of arachidonic acid from phospholipids cell membranes. Directions for use and doses

Pharmacological properties
Pharmacodynamics
It has anti-inflammatory, antipruritic, antiexudative and antiallergic effects.
Inhibits the release of inflammatory mediators. When exposed to the skin, the marginal accumulation of neutrophils is prevented, which leads to a decrease in inflammatory exudate and cytokine production; inhibition of macrophage migration, reduction of infiltration and granulation processes.

Pharmacokinetics
Fluocinolone acetonide easily penetrates the skin through the stratum corneum. It does not undergo biotransformation in the skin and accumulates in the stratum corneum. It is detected in the skin even 15 days after its topical application. When fluocinolone is used externally, acetonide may be absorbed to a small extent and cause general symptoms. After absorption, it is metabolized mainly in the liver. In the form of compounds with glucuronic acid and in a small amount in an unbound form, it is excreted through the kidneys and slightly with bile.

Indications for use
Inflammatory diseases skins amenable to glucocorticosteroid therapy:

  • atopic dermatitis;
  • eczema;
  • seborrheic dermatitis;
  • psoriasis, including the scalp;
  • exudative erythema multiforme;
  • lichen planus.

Contraindications
Hypersensitivity, skin manifestations of syphilis, skin tuberculosis, diaper rash, rosacea, extensive psoriatic plaques, anogenital itching;
skin infections caused by bacteria, viruses, fungi; wound damage to the skin, ulcers; erosive and ulcerative lesions of the gastrointestinal tract;
tumors and precancerous skin conditions; pregnancy, lactation period, childhood(up to 2 years).

Directions for use and doses
Externally.
A small amount of the drug is applied to the skin, previously wiped with a swab moistened with an antiseptic liquid, 2-4 times a day and lightly rubbed. The duration of treatment depends on the nature of the disease and is usually 5-10 days. If necessary, an occlusive dressing can be applied. It is not allowed to use more than 2 g/day under a bandage.
Flucinar ® in the form of a gel is indicated in the first phase of treatment (before ointment) and for the treatment of diseases of the scalp, as well as in that category of patients who do not tolerate the drug well due to the ointment base.
The gel form makes it possible to apply it in small quantities, and therefore smaller doses of the drug over a larger surface of the skin.

Side effect
Burning, itching, steroid acne, stretch marks, dry skin, folliculitis. With long-term use - skin atrophy, local hirsutism, hypertrichosis, telangiectasia, purpura, skin pigmentation disorders, alopecia, especially in women, secondary immunodeficiency (exacerbation of chronic infectious diseases, generalization of the infectious process, development of opportunistic infections); when applied to large surfaces, systemic manifestations are possible (gastritis, “steroid” gastric ulcer, adrenal insufficiency, Itsenko-Cushing syndrome, “steroid” diabetes, slowing down reparative processes).

Overdose
Overdose for external use is very rare.
Symptoms: itching and burning of the skin at the site of application of the drug, hyperglycemia, glucosuria, Itsenko-Cushing syndrome.
Treatment: symptomatic with gradual withdrawal of the drug.

Interaction with other drugs
Compatible with antimicrobial agents.
Reduces the activity of antihypertensive, diuretic, antiarrhythmic drugs medicines, K+ drugs. Diuretic drugs (except potassium-sparing drugs) increase the risk of hypokalemia.
During treatment with glucocorticosteroids, vaccinations and immunizations should not be performed due to the immunosuppressive effect of the drug.

special instructions
If secondary infections occur at the site of application of the drug, an antibacterial or antifungal treatment.
Use Flucinar ® with extreme caution on the skin of the face, since absorption of the drug increases and the possibility of side effects increases.
Use with caution in persons with existing atrophic skin changes, especially in the elderly.
It is recommended to mow loose clothing during treatment.

Impact on ability to manage vehicle and maintenance of moving mechanisms.
The drug does not limit psychophysical activity, the ability to drive a vehicle or operate moving machinery.

Release form
Gel for external use 0.025%.
15 g each in aluminum tubes with lacquered lithographed inside.
The tube with instructions for use is placed in a lithographed cardboard box.

Best before date
3 years.
Do not use after expiration date.

Storage conditions
Store at a temperature not exceeding 25°C. Do not freeze.
Keep out of the reach of children.
