1. This procedure regulates the issues of submitting information about patients necessary for the formation of the Federal Register of persons with hemophilia, cystic fibrosis, pituitary dwarfism, Gaucher disease, malignant neoplasms of lymphoid, hematopoietic and related tissues, multiple sclerosis, persons after organ transplantation and (or) tissues (hereinafter referred to as the Federal Register), as well as information on dispensed drugs provided for in the list of drugs purchased from the federal budget intended for the treatment of patients with malignant neoplasms of lymphoid, hematopoietic and related tissues, hemophilia, cystic fibrosis, pituitary dwarfism, Gaucher disease , multiple sclerosis, as well as after organ and (or) tissue transplantation, approved by order of the Government of the Russian Federation of December 31, 2008 N 2053-r 1 (hereinafter referred to as the list of drugs).

2. When establishing for the first time the diagnosis specified in paragraph 1 of this procedure, medical organizations of the constituent entities of the Russian Federation, medical organizations municipal system health care in the event of transfer to them, in the prescribed manner, of the powers of state authorities of the constituent entities of the Russian Federation in the field of health care, in which patients are receiving medical care, and medical organizations under the jurisdiction of Federal service execution of punishment (FSIN of Russia), are submitted no later than five working days from the date of diagnosis on paper and (or) in in electronic format to the authorized executive body of a constituent entity of the Russian Federation, a referral to include information (amend information) about the patient (hereinafter referred to as the referral) in the Federal Register of Persons with Hemophilia, Cystic Fibrosis, Pituitary Dwarfism, Gaucher Disease, Malignant Neoplasms of Lymphoid, Hematopoietic and Related Tissues , multiple sclerosis, persons after organ and (or) tissue transplantation, in form N 01-FR according to Appendix N 2.

Medical organizations under the jurisdiction of the Federal Medical and Biological Agency (hereinafter referred to as the FMBA of Russia) submit information in accordance with paragraph one of paragraph 2 of this Procedure to the FMBA of Russia.

3. Notices about the exclusion of information about patients from the Federal Register (hereinafter referred to as the notice) are submitted by medical organizations of the constituent entities of the Russian Federation, medical organizations of the municipal healthcare system in the event of transfer to them in the prescribed manner of powers of state authorities of the constituent entities of the Russian Federation in the field of health protection and by medical organizations, under the jurisdiction of the Federal Penitentiary Service of Russia, to the authorized executive body of the constituent entity of the Russian Federation on paper and (or) in electronic form within five working days from the date of receipt of such information in Form No. 02-FR in accordance with Appendix No. 3.

Medical organizations under the jurisdiction of the FMBA of Russia provide information in accordance with paragraph one of clause 3 of this Procedure to the FMBA of Russia.

4. Referrals and notices are registered by medical organizations in a journal in Form N 03-FR in accordance with Appendix No. 4.

5. The authorized executive body of the constituent entity of the Russian Federation and the FMBA of Russia, for the formation of the Federal Register, within five working days from the date of receipt of the referral and (or) notification from medical organizations, submit information on paper and (or) electronically to the Ministry of Health of the Russian Federation about the patient in form N 04-FR in accordance with Appendix No. 5.

Information about the patient (referrals, notices) submitted electronically must be certified by an electronic signature official authorized executive body of a constituent entity of the Russian Federation or FMBA of Russia.

6. Pharmacy organizations, the list of which is approved by the authorized executive body of the constituent entity of the Russian Federation, and medical organizations under the jurisdiction of the FMBA of Russia, monthly no later than the 10th day of the month following the reporting month, submit to the authorized executive authorities of the constituent entities of the Russian Federation or FMBA of Russia, respectively, information about dispensed medicinal products provided for in the list of medicinal products, on paper and in electronic form in form N 05-FR in accordance with Appendix No. 7.

Application. The procedure for providing medical care to adult patients with infectious diseases

Application

1. This Procedure regulates the provision of medical care to adult patients with infectious diseases (hereinafter referred to as patients with infectious diseases) in medical organizations, with the exception of the provision of medical care to adult patients with a disease caused by the human immunodeficiency virus (HIV infection).

2. Medical care for patients with infectious diseases is provided in the form of emergency, primary health care and specialized medical care in medical organizations and their structural divisions operating in accordance with Appendices No. 1-8 to this Procedure.

3. Within the framework of emergency medical care, including emergency specialized medical care, medical care for patients with infectious diseases is provided by paramedic mobile ambulance teams, medical mobile emergency medical teams, specialized mobile emergency medical teams and includes measures to eliminate life-threatening conditions followed by medical evacuation to a medical organization providing inpatient medical care to patients with infectious diseases.

4. Patients with infectious diseases that do not pose a danger to others, mild degree or if such diseases are suspected, medical care is provided in the form of primary health care on an outpatient basis by general practitioners, local physicians, general practice and medical specialists who carry out a set of therapeutic and diagnostic measures, including those aimed at identifying the causative agent of infectious diseases and carrying out primary anti-epidemic measures carried out medical workers medical organization.

5. Specialized care for patients with infectious diseases is provided in medical organizations or their structural divisions that provide specialized medical care, including in infectious diseases departments of multidisciplinary hospitals and infectious diseases hospitals.

6. The provision of medical care in inpatient conditions to patients with infectious diseases is carried out according to medical indications - in cases of severe and moderate course of the infectious disease, the inability to establish a diagnosis on an outpatient basis, the need for additional laboratory and instrumental methods research to conduct differential diagnosis, absence clinical effect from the therapy performed on an outpatient basis, as well as for epidemiological indications in accordance with current sanitary legislation.

Treatment of patients with infectious diseases in a hospital setting is carried out under the direction of a general practitioner, a local physician, or a general practitioner ( family doctor), emergency physician, infectious disease doctor, medical specialists who have identified an infectious disease.

Providing medical care in a hospital setting to patients with infectious diseases in the presence of medical indications specified in paragraph one of this paragraph is also possible upon self-referral of a patient with infectious diseases.

7. Medical care for patients with infectious diseases with life-threatening acute conditions, including infectious-toxic, hypovolemic shock, cerebral edema-swelling, acute renal and liver failure, acute cardiovascular and respiratory failure, it turns out:

outside a medical organization - emergency medical care teams (including intensive care units);

in stationary conditions - in boxes, intensive care wards (blocks), intensive care units multidisciplinary hospital, as well as in intensive care wards (blocks), intensive care units of infectious diseases hospitals in compliance with established sanitary and anti-epidemic standards.

8. The provision of medical care to patients with infectious diseases in combination with diseases of other organs is carried out taking into account the recommendations of medical specialists of the relevant profile (obstetricians-gynecologists, urologists, ophthalmologists, coloproctologists, gastroenterologists, surgeons, other medical specialists). The provision of medical care to pregnant women with infectious diseases is carried out taking into account the recommendations of an obstetrician-gynecologist in observational departments of maternity hospitals or in hospitals of medical organizations providing medical care to patients with infectious diseases.

9. Discharge of patients with infectious diseases from the hospital of a medical organization is carried out in accordance with sanitary and epidemiological rules after completion of the course of treatment and control tests laboratory research. Convalescents of infectious diseases are subject to dispensary observation.

10. Dispensary observation and treatment of patients who have suffered infectious diseases, as well as treatment of patients with infectious diseases in the convalescence stage is carried out in the department (office) of infectious diseases of a medical organization providing primary health care, or its structural divisions and structural divisions of infectious diseases hospitals, providing outpatient medical care.

11. Information about an identified case of disease is sent by the medical organization to territorial body, authorized to carry out sanitary and epidemiological surveillance at the place of registration of the disease, within 2 hours from the moment of diagnosis (by telephone), and then within 12 hours (in writing) on ​​the form emergency notice.

A medical organization that has changed or clarified the diagnosis, within 12 hours in writing, using the form of an emergency notification, informs the territorial body authorized to carry out sanitary and epidemiological surveillance at the place of registration of the disease about the changed (clarified) diagnosis, the date of its establishment, and the original diagnosis.

1. These Regulations determine the organization of activities of the infectious diseases department of a multidisciplinary hospital (infectious diseases hospital).

2. The infectious diseases department of a multidisciplinary hospital (infectious diseases hospital) (hereinafter referred to as the Department) is a structural unit of a medical organization that provides specialized medical care to patients with infectious diseases in a hospital setting in accordance with approved standards of medical care.

3. The department can be mixed (for hospitalization of patients with various infections) or specialized (for hospitalization of patients with a specific infection).

4. The department must have at least 50% of the total number of beds in boxed wards.

5. The structure and staffing level of the medical and other personnel of the department are determined based on the volume of diagnostic and treatment work carried out and the number of the population served, taking into account the recommended staffing standards for the medical and other personnel of the department in accordance with Appendix No. 5 to the Procedure for providing medical care to patients with infectious diseases, approved by this by order.

1. These Regulations determine the organization of activities of an infectious diseases hospital.

2. An infectious diseases hospital is a medical organization that provides medical care to patients with infectious diseases.

3. The structure and staffing level of medical and other personnel of an infectious diseases hospital of the state and municipal health care system are established depending on the volume of medical care provided, taking into account the recommended staffing standards for medical and other personnel of an infectious diseases hospital in accordance with Appendix No. 8 to the Procedure for providing medical care to patients with infectious diseases, approved by this order.

4. A specialist is appointed to the position of chief physician of an infectious diseases hospital who meets the Qualification characteristics of positions of workers in the healthcare sector, approved by order of the Ministry of Health and Social Development of Russia dated July 23, 2010 N 541n (registered by the Ministry of Justice of Russia on August 25, 2010, registration N 18247).

5. The equipment of the infectious diseases department of an infectious diseases hospital is carried out in accordance with the standard of equipment for the infectious diseases department of a multidisciplinary medical organization, an infectious diseases hospital, installed application N 6 to the Procedure for providing medical care to adult patients with infectious diseases, approved by this order, equipping specialized departments (offices) for providing individual species Medical care (by profile) for patients with infectious diseases is carried out in accordance with equipment standards, established procedures for providing certain types of medical care (by profile), as well as sanitary and epidemiological rules.

1. This Procedure regulates the provision of medical care to adult patients with infectious diseases (hereinafter referred to as patients with infectious diseases) in medical organizations, with the exception of the provision of medical care to adult patients with a disease caused by the human immunodeficiency virus (HIV infection).

2. Medical care for patients with infectious diseases is provided in the form of emergency, primary health care and specialized medical care in medical organizations and their structural divisions operating in accordance with this Procedure.

3. Within the framework of emergency medical care, including emergency specialized medical care, medical care for patients with infectious diseases is provided by paramedic mobile ambulance teams, medical mobile emergency medical teams, specialized mobile emergency medical teams and includes measures to eliminate life-threatening conditions followed by medical evacuation to a medical organization providing inpatient medical care to patients with infectious diseases.

4. Patients with infectious diseases that do not pose a danger to others, of a mild degree, or if such diseases are suspected, are provided with medical care in the form of primary health care on an outpatient basis by general practitioners, local physicians, general practitioners and medical specialists, who carry out a set of therapeutic and diagnostic measures, including those aimed at identifying the causative agent of infectious diseases and carrying out primary anti-epidemic measures carried out by medical workers of a medical organization.

5. Specialized care for patients with infectious diseases is provided in medical organizations or their structural divisions that provide specialized medical care, including in infectious diseases departments of multidisciplinary hospitals and infectious diseases hospitals.

6. The provision of medical care in inpatient conditions to patients with infectious diseases is carried out according to medical indications - in cases of severe and moderate course of the infectious disease, the inability to establish a diagnosis on an outpatient basis, the need for additional laboratory and instrumental research methods for differential diagnosis, lack of clinical effect from the therapy performed on an outpatient basis, as well as for epidemiological indications in accordance with current sanitary legislation.

Treatment of patients with infectious diseases in a hospital setting is carried out on the direction of a general practitioner, local physician, general practitioner (family doctor), emergency physician, infectious disease specialist, or medical specialists who have identified an infectious disease.

Providing medical care in a hospital setting to patients with infectious diseases in the presence of medical indications specified in paragraph one of this paragraph is also possible upon self-referral of a patient with infectious diseases.

7. Medical care for patients with infectious diseases with life-threatening acute conditions, including infectious-toxic, hypovolemic shock, cerebral edema-swelling, acute renal and liver failure, acute cardiovascular and respiratory failure, is provided:

In stationary conditions - in boxes, intensive care wards (blocks), intensive care units of a multidisciplinary hospital, as well as in intensive care wards (blocks), intensive care units of an infectious diseases hospital in compliance with established sanitary and anti-epidemic standards.

8. The provision of medical care to patients with infectious diseases in combination with diseases of other organs is carried out taking into account the recommendations of medical specialists of the relevant profile (obstetricians-gynecologists, urologists, ophthalmologists, coloproctologists, gastroenterologists, surgeons, others medical specialists). The provision of medical care to pregnant women with infectious diseases is carried out taking into account the recommendations of an obstetrician-gynecologist in observational departments of maternity hospitals or in hospitals of medical organizations providing medical care to patients with infectious diseases.

9. Discharge of patients with infectious diseases from the hospital of a medical organization is carried out in accordance with sanitary and epidemiological rules after completion of the course of treatment and control laboratory tests. Convalescents of infectious diseases are subject to dispensary observation.

10. Dispensary observation and treatment of patients who have had infectious diseases, as well as treatment of patients with infectious diseases in the convalescence stage is carried out in the department (office) of infectious diseases of a medical organization providing primary health care or its structural divisions and structural divisions of infectious diseases hospitals providing outpatient medical care.

11. Information about the identified case of the disease is sent by the medical organization to the territorial body authorized to carry out sanitary and epidemiological surveillance at the place of registration of the disease, within 2 hours from the moment of diagnosis (by phone), and then within 12 hours (in writing) using an emergency form notices.

A medical organization that has changed or clarified the diagnosis, within 12 hours in writing, using the form of an emergency notification, informs the territorial body authorized to carry out sanitary and epidemiological surveillance at the place of registration of the disease about the changed (clarified) diagnosis, the date of its establishment, and the original diagnosis.

1. These Regulations regulate the activities of the department (office) of infectious diseases of a medical organization providing primary health care (hereinafter referred to as the Department (office).

2. The department (office) of infectious diseases is a structural unit of a medical organization or a unit of a medical organization providing primary health care in an outpatient setting.

The procedure for providing medical care to adult patients with infectious diseases (with the exception of HIV infection) has been approved.

Patients can be provided with emergency, primary health care and specialized medical care. Regulations on the organization of activities have been determined medical institutions and them structural divisions, in which the specified assistance is provided. We are talking about an infectious diseases hospital, an infectious diseases department of a multidisciplinary hospital, as well as a department (office) of infectious diseases of a medical organization providing primary health care. The standards for their equipment and recommended staffing standards for medical and other personnel are given.

Emergency (including specialized) care is provided by paramedics, doctors and specialized mobile ambulance teams. They take measures to eliminate life-threatening conditions and deliver patients to a medical facility.

Patients who do not pose a danger to others, mild cases, or those with suspected infectious diseases are provided with primary health care on an outpatient basis.

Treatment of patients in a hospital setting is possible with the referral of a therapist, general practitioner (family doctor), infectious disease specialist, or other medical specialists who have identified an infectious disease.

After completing the course of treatment and control laboratory tests, patients are discharged.

An electronic notification is sent to the territorial body that carries out sanitary and epidemiological supervision about identified infectious diseases.
