Veroshpiron indications contraindications. Veroshpiron is the first diuretic to replenish potassium levels. Indications for use Veroshpiron

The health status of many patients requires the use of diuretics. The family of these drugs is quite large, but very often doctors still prefer veroshpiron. What do these pills help with, what is their advantage, how is this medicine taken and why should you not take it for weight loss?

Veroshpiron and its difference from other diuretics

Diuretics have one unpleasant quality: when they are taken along with excess fluid, potassium is also lost. Veroshpiron has no such drawback. This is a high-quality and effective potassium-sparing diuretic, the basic component of which is spironolactin.

The mechanism of action of the drug is quite complex. It is enough for patients to know that its active substance acts on the adrenal cortex, blocking the formation of the hormone aldosterone (namely, it promotes the removal of potassium during water retention). As a result, fluid leaves the body along with sodium and chlorine, but potassium reserves are not depleted.

The use of Veroshpiron tablets helps eliminate swelling, reduce blood pressure, and improve blood circulation. It is produced in capsules (the proportion of active ingredient in them is 50 or 100 mg) and tablets (25 mg). The diuretic effect when taking the drug is observed after 2-5 days. Spironolactin is completely absorbed from the gastrointestinal tract (it is also able to pass through the placenta and enter breast milk) and is excreted from the body in urine (60%) and feces (40%).

What will this medicine help with?

Veroshpiron is usually prescribed by endocrinologists. This is what these pills are prescribed for:

  • edema caused by heart failure;
  • disturbances in the functioning of endocrine organs (multiple cysts on the ovaries, hirsutism, Conn's disease);
  • arterial hypertension;
  • ascites (with liver cirrhosis);
  • cerebral edema;
  • paroxysmal myoplegia caused by calcium deficiency;
  • myasthenia gravis;
  • nephrotic syndrome;
  • massive burns;
  • swelling during pregnancy.

Since self-prescription of the medicine is not allowed, the doctor selects the regimen for taking it. The patient should not reduce or increase the dose himself. The standard daily dosages prescribed by the instructions for the use of Veroshpiron tablets for various pathologies are as follows:

  • cirrhosis – from 100 to 400 mg;
  • nephrotic syndrome – 100-200 mg;
  • swelling – 100-200 mg;
  • hypertension – from 50 to 100 mg 1-4 r. per day 14 days;
  • polycystic ovary syndrome and high levels of male hormones in a woman’s body – 100 mg 2 times. per day;
  • children over 3 years of age – from 30 to 90 mg once or in 4 doses.

The medicine is taken 3-4 times. per day every day during or immediately after meals for 5 days, and the dose is either gradually reduced and increased to 25 mg, or increased to 400 mg. Then drink 1 tablet. 4 rub. per day with an interval of 3 days.

Possible side effects

Despite the proven effectiveness and ability of Veroshpiron not to deplete potassium reserves, it can cause quite a few unwanted reactions. Here are the negative manifestations that are sometimes accompanied by taking this drug:

  • loose stools or constipation;
  • pressure drop;
  • dehydration;
  • hyperkalemia;
  • heart rhythm disturbance;
  • inflammation of the stomach;
  • intestinal colic;
  • ulcerative damage to the gastrointestinal tract and internal bleeding;
  • nausea;
  • stomach ache;
  • vomit;
  • dizziness;
  • lethargy, drowsiness;
  • changes in blood composition (thrombocytopenia, decreased leukocyte levels, increased creatinine concentration);
  • headache;
  • muscle cramps;
  • movement coordination disorder;
  • increased uric acid levels;
  • megaloblastosis (proliferation of bone marrow cells);
  • a painful condition characterized by immobility and lack of response to external stimuli (lethargy).

With prolonged use of the drug, men may develop gynecomastia (enlargement of mammary gland tissue) and decreased potency. And in women - menstrual disorder and complete absence of menstruation, pathologies of the mammary glands, deepening of the voice, male pattern hair growth, allergic manifestations.

Caution: when is it better to refrain from taking the drug?

Even if there are indications for taking Veroshpiron, but the patient has an excess of potassium, sodium deficiency, kidney problems, allergies, then you should stop using it. This diuretic is not prescribed during breastfeeding (if this is not possible, then the baby will have to be transferred to IV). The question of the advisability of its use by a pregnant woman is decided by the doctor.

Contraindications also include:

  • diabetes;
  • adrenal insufficiency;
  • Addison's disease;
  • reduction or complete cessation of urination;
  • first trimester of pregnancy;
  • liver failure;
  • disturbance of acid-base balance in the body (metabolic acidosis).

Particular caution should be exercised when prescribing Veroshpiron to elderly people, women with dysmenorrhea, as well as during local anesthesia and general anesthesia.

Advise or dissuade: what conclusions do patients draw?

Reviews testify in favor of diuretic tablets Veroshpiron. People confirm its effectiveness. This diuretic fully copes with its purpose: swelling goes away within 2-3 days after taking it, blood pressure returns to normal. Many people pay attention to this fact: in parallel with this, weight decreases. By removing excess fluid, some lose 5 kg in just a week. But such a medicine should under no circumstances be used as a means to gain weight, as it can cause kidney failure.

Among the reviews there are also negative ones. Quite often the drug provokes side effects. In addition, people do not like the fact that they have to wait for the cumulative effect and limit themselves in nutrition. After all, foods containing potassium are prohibited: apricots, oranges, prunes, bananas, tomatoes.

Veroshpiron An effective and affordable diuretic. When taken correctly, it is a very good helper for those who suffer from edema. It is not as aggressive as its “competitors”; such a medicine will not take away precious potassium from the heart. But before you buy it, you should definitely ask your doctor whether such a drug is necessary in each specific case.

The international nonproprietary name of the drug is Spironolactone. Veroshparon is a potassium-sparing diuretic. It acts as a competitive antagonist of aldosterone (adrenal hormone). Veroshpiron is a non-standard diuretic. Unlike conventional diuretics, it does not remove potassium, but promotes its accumulation in the body.

pharmachologic effect

Although the drug has a long history of medical use and has proven itself positively, it should only be taken as prescribed by a medical specialist. Too much potassium is just as dangerous for the body as too little potassium. The mechanism of action of Veroshpiron is based on the competition of its active substance with aldosterone. When binding to receptors sensitive to aldosterone, fluid is removed from the body. At the same time, sodium, chlorine, potassium and urea are not excreted after taking the drug and can accumulate in the body. These processes cause a decrease in urine acidity.

The most pronounced effect of taking the drug can be observed 7 hours after oral administration. The total duration of action is at least 24 hours. The diuretic effect determines the hypotensive effect of Veroshpiron. Pronounced diuretic effect manifests itself on days 2–5 of taking the drug.

When taken orally, the medicine is completely and quickly absorbed from the gastrointestinal tract, binds to blood plasma proteins (approximately 98%), then reaches the liver, where it breaks down into metabolic products (metabolites). In the liver it is converted into active metabolites:

  • metabolite containing sulfur (80%),
  • metabolite containing canrenone (20%).

The drug penetrates poorly into tissues and organs, but it itself and its metabolites penetrate the placental barrier, and canrenone passes into breast milk. The half-life of veroshpiron is 13–24 hours, and its active metabolites - up to 15 hours. It is excreted mainly by the kidneys: 50% - in the form of metabolites, 10% - unchanged and partially. The remaining amount is excreted from the gastrointestinal tract with feces.

If the patient has cirrhosis of the liver or heart failure, the half-life after taking the drug becomes longer. In this case, there are no signs of cumulation. Its likelihood increases for patients with chronic renal failure and hyperkalemia.

Indications for use of Veroshpiron

  • high blood pressure and stagnation of fluid in the body, which provoke the development of heart disease, kidney disease and lung problems;
  • lack of potassium and magnesium in the body, combined with other indications for taking diuretics;

Instructions for use

Veroshpiron is taken orally at the same time as a meal or immediately after it. Regarding the dosage and duration of use, you must follow the instructions received from your doctor or the instructions for the drug.

Usually the drug is prescribed to be taken 2 times a day. If the drug is prescribed once a day, it is better to take it in the morning; if 2 times a day, then in the morning and at lunch. Use of the drug in the evening is not recommended, as it may lead to too frequent visits to the toilet.

Your doctor may change the dosage, frequency, and duration of use based on your body's response to the drug. The official instructions for use provide the following regimens for taking medication for various diseases and conditions.

Veroshpiron for essential hypertension

The standard dose prescribed for adults per day is usually 50-100 mg. An increase to 200 mg is possible, but this process must be carried out gradually. To obtain pronounced and lasting results from therapy, the drug must be taken for at least 2 weeks. It is possible to adjust the dose if necessary.

Veroshpiron for idiopathic hyperaldosteronism

The instructions for use note that the daily dose of the drug is from 100 to 400 mg. For severe hyperaldosteronism and hypokalemia: the daily dose is 300 mg (possibly increased to 400 mg), divided into 2-3 doses. As the patient's condition improves, the dose is gradually reduced to 25 mg per day.

Veroshpiron for hypokalemia/hypomagnesemia

If these diseases are caused by diuretic therapy, then the drug is prescribed at a dose of 25-100 mg of medication per day. The dose can be taken once or divided into several doses. In cases where oral potassium supplements or other methods of replenishing it are ineffective, it is possible to increase the maximum daily dose of the drug to 400 mg.

Veroshpiron for primary hyperaldosteronism

For a short diagnostic test: 400 mg daily is taken for 4 days. This amount is distributed over several doses per day. If potassium levels in the blood increase while taking the drug and decrease after discontinuation, the presence of primary hyperaldosteronism can be assumed.

For a long-term diagnostic test: 400 mg per day is taken for 3-4 weeks. A short course of preoperative therapy with Veroshpiron for primary hyperaldosteronism. After diagnosing hyperaldosteronism, the medicine should be taken in a daily dosage of 100-400 mg. This dose is used during the entire period of preparation for surgery, taken 1-4 times a day. When surgery is not indicated, the drug is taken as a means of long-term maintenance therapy. In this case, the doctor prescribes the smallest effective dose; it is selected for each patient individually.

Veroshpiron for edema due to nephrotic syndrome

The daily dose is 100-200 mg. Taking the drug is recommended only in cases of ineffectiveness of other types of therapy (since the effect of spironolactone on the underlying pathological process has not been proven). If edema is observed against the background of chronic heart failure, then the drug is prescribed for 5 days, 100-200 mg, divided into 2-3 doses. Veroshpiron is prescribed together with a loop or thiazide diuretic. The daily dose as prescribed by a doctor can be reduced to 25 mg.

If edema is observed against the background of liver cirrhosis, then the daily dose for adults is 100 mg (with the ratio of sodium and potassium ions (Na+/K+) in the urine exceeding 1.0). In other cases, the daily dose of the drug for adults is prescribed at 200-400 mg. For children, the initial dose is 1-3.3 mg per 1 kg of body weight. Reception is carried out 1-4 times.

After 5 days, the dose is adjusted, possibly increasing 3 times compared to the original. If the next dose of the drug is missed, but no more than 4 hours have passed, you should immediately take the missed dose of the drug. Otherwise, take Veroshpiron in the usual dosage at your next dose.

During therapy with Veroshpiron, you should avoid consuming excessive amounts of salt and potassium-rich foods (bananas, apricots, coconuts, peaches, dates, oranges, tomatoes, grapefruits, prunes). You cannot drink alcoholic beverages. At the beginning of treatment with the drug, you must stop driving vehicles and other activities that require quick reactions and increased attention. The duration of restrictions while taking the drug will depend on the patient’s health condition.

Veroshpiron for weight loss

Veroshpiron, as a diuretic drug, can get rid of several extra pounds by removing fluid from the body. Regimen for taking weight loss medications are published on the Internet and in some periodicals. Despite the fact that the product can have such an effect, taking it for weight loss is strictly prohibited. Loss of body weight does not occur as a result of loss of adipose tissue, and the fluid removed from the body is easily replaced in the coming days. Therefore, the effect is short-lived. It may be accompanied by a number of serious side effects. Taking medication, as well as other diuretics for weight loss, is absolutely unjustified and even dangerous!

Veroshpiron for children

Despite the fact that children under 3 years of age are indicated in the instructions as contraindications, in practice Veroshpiron is sometimes prescribed to younger children (even infants). But treatment of children of this age must necessarily be carried out in a hospital or at least under strict medical supervision. In pediatric practice, this drug can be taken as a diuretic for various diseases. The duration of use and dosage are determined only by the doctor; parents must carefully and thoroughly follow the doctor's instructions.

It is allowed to grind the tablet into powder and mix it with milk or baby food. In some cases, especially in infants, vomiting occurs after use. If half an hour has not passed after taking the medication Veroshpiron, the child should be given another dose of the drug. If more than half an hour has passed, then there is no need to give the drug additionally.

An overdose of the drug in children, especially young children, is very dangerous. It is accompanied by drowsiness, weakness, and possibly heart rhythm disturbances or seizures. Signs of dehydration are detected, vomiting or diarrhea is possible. In these cases, you should urgently stop taking the drug and call a doctor or an ambulance.

During pregnancy and lactation

Veroshpiron should not be taken during pregnancy and lactation. If this drug is prescribed to nursing mothers according to indications, then breastfeeding should be discontinued, since spironolactone can pass into milk and negatively affect the child’s body.

Overdose

Symptoms of an overdose of Veroshpiron include: nausea, vomiting, decreased blood pressure, diarrhea, dizziness, hyperkalemia, hyponatremia, hypercalcemia, dehydration, rash, increased urea concentration. Overdose therapy should be aimed at eliminating symptoms. It consists of gastric lavage, treatment of dehydration and hypotension. In case of severe overdose symptoms, hemodialysis is performed after taking the drug.

Interaction with other drugs

Veroshpiron can interact with many medications, so when prescribing it, you should inform your doctor about taking any medications, vitamins and dietary supplements.

Instructions for use

Veroshpiron is not used in therapeutic practice as an independent remedy, but plays an important role in the treatment of certain diseases.

Indications for the use of Veroshpiron are conditions in which it is necessary to remove excess fluid from the body, but at the same time maintain water and electrolyte balance.

Veroshpiron is also used in the diagnosis of primary hyperaldosteronism.

Method of administration

An incorrectly selected dosage of the drug can lead to a deterioration in the patient’s condition due to a lack of therapeutic effect or an overdose.

The amount and frequency of taking the medicine must completely coincide with the values recommended by the attending physician.

For EG, the initial dose of Veroshpiron is 50-100 mg per day. According to indications, the dosage is increased to 200 mg per day by adding 50 mg of the drug once every 15 days.

To achieve a lasting therapeutic effect, it is recommended to take the medicine for at least 2 weeks. Repeated dose adjustments may be required during treatment.

For hyperaldosteronism of unknown origin, drugs are prescribed in a daily dosage of 100 to 400 mg, depending on the severity of the symptoms of the disease.

In case of a complicated course of the disease against the background of developed hypokalemia, the maximum daily dosage is 400 mg.

The drug is taken three times a day in equal portions. If the dynamics are positive, the dosage is reduced to 25 mg per day.

For hypomagnesemia and hypokalemia, the development of which is provoked by taking diuretics, Veroshpiron is prescribed in a daily dose of 25 to 100 mg, and only if drugs with potassium are not effective.

For swelling, the drug is prescribed in a dosage of 100 or 200 mg. If edema appears due to CHF, the dosage remains the same, but the duration of therapy is 5 days, and the daily dose of the drug is divided into 2 or 3 doses.

After the patient's condition improves, the dosage can be reduced to 25 mg.

In preparation for surgical treatment of Conn's syndrome, Veroshpiron is prescribed in a dose of 100 to 400 mg, divided into 1-4 doses. In case of cancellation of the operation, an individual maintenance dose is selected at the lowest value.

For the treatment of edema in children, the drug is prescribed in a dosage calculated according to the formula 1-3 mg/kg of the child’s weight per day, divided into several doses (their maximum can be 4).

If necessary, after 5 days from the start of the course, the dosage can be increased, but not more than 3 times the original.

Arrhythmia is a violation of cardiac activity, deviations in the rhythm of heart contractions that are different in nature and origin. Find out more about symptoms and treatment:

Release form, composition

Veroshpiron is produced in the form of tablets and soft-shell capsules. The main active ingredient of both dosage forms is the compound spironolactone.

One tablet contains 25 mg, and a capsule contains 50 or 100 mg. In addition to the main component, the tablets contain additional (stabilizing) components.

They are also found in the contents of gelatin capsules.

Since the active substance of Veroshpiron enters the systemic bloodstream, the drug may interact with medications that are taken simultaneously with it.

When used together with direct and indirect anticoagulants, as well as with cardiac glycosides, a decrease in the toxicity of these drugs is observed.

The opposite effect is observed when taking Veroshpiron together with lithium preparations.

When taken together with glucocorticosteroids and diuretics based on furosemide, benzothiazine and ethacrynic acid, the diuretic effect increases, against which there is an increase in the loss of sodium ions. An increase in the effect of the drugs is also observed when taking antihypotensive drugs together with Veroshpiron.

When taken together with phenazoles, triptorelin and gonadorelin, the effect of the latter is enhanced due to their increased metabolism. At the same time, their half-life from the body is shortened.

Veroshpiron can significantly reduce the effectiveness of norepinephrine and mitotane. If it is necessary to use them together, dosage adjustment is recommended.

Indomethacin and salicylates significantly reduce the effect of Veroshpiron.

Side effects

During the course of treatment with Veroshpiron, adverse reactions may occur, many of which are not a reason to discontinue the drug:

  • dyspeptic disorders, pain in the abdominal area;
  • sleep disturbances, lethargy, dizziness;
  • changes in laboratory tests of blood and other body fluids;
  • endocrine disruptions (in women - deepening of the voice, metrorrhagia, amenorrhea or dysmenorrhea, in men - gynecomastia, as well as decreased libido and potency);
  • muscle spasms.

In rare cases, more severe side effects occur:

  • bleeding in the gastrointestinal tract;
  • confusion, hallucinations and lethargy;
  • allergic skin reactions;
  • acute renal failure;
  • If they occur, it may be necessary to discontinue the drug or review the dosage and reduce it.

Contraindications

  • hyperkalemia and hypernatremia;
  • severe form of renal failure;
  • intolerance to the components of the drug, including lactose;
  • Addison's disease.

Taking this drug is relatively dangerous for atrioventricular block, diabetes mellitus and diabetic nephropathy, cirrhosis of the liver. For these pathologies, the drug is prescribed only under round-the-clock supervision of medical staff.
